Required Forms

When a parent visits the school to register a student, the registrar or other school official will provide the parent with the following forms:

1. Student Registration Form (SAMPLE form, for viewing only)*
*NOTE: The SAMPLE FORM above provides a preview of the information required for student registration. DO NOT COMPLETE THIS SAMPLE FORM. You must obtain an official copy of the Registration Form from a school office.
2. Home Language Survey
(Required for all students)
